The Primary Health Care Corporation (PHCC), in collaboration with the Ministry of Education and Higher Education, has introduced an Electronic Student Health Declaration Form to help ensure that school health teams have up-to-date health information about students.Parents are requested to complete the electronic form for their children, providing essential health details that will help school health teams offer appropriate care and respond more effectively to students’ health needs, including in emergency situations. The form can be accessed online using the child’s Qatar ID (QID) and a verification code. PHCC has also confirmed that students’ health information will be kept strictly confidential. Parents are encouraged to complete the form to support their child’s health and wellbeing throughout the school year.
